Introducing our provider credentialing checklist
According to our research, credentialing takes facilities an average of 20 hours per physician.
This process can take longer if something is missing from the provider’s application or facilities aren’t organized. Even if you have a general idea of what needs to be done, having it listed out can make the process much easier and faster.
That’s why we created a provider credentialing checklist
What’s in the checklist?
For this checklist, we reached out to multiple facility managers and researched other provider credentialing checklists. We consolidated all of that information into a single checklist.
To make it more digestible, the checklist is separated into three sections:
- Background & verification
- Delineation of privileges
Each section has two or three big checkboxes that can be marked after you’ve completed and checked the tasks underneath them. The checklist starts with requesting application information and ends with providing an appointment letter—covering the entire process from start to finish.
How does the checklist help?
Think of our provider credentialing checklist like a blueprint.
The checklist shows you everything you should need to fully credential a provider. It’s organized and has empty boxes, so you can track your progress along the way. Once everything is checked, you can rest easy knowing your new provider is ready to start at your facility.
Our checklist makes the process easier even if you credential manually, but if you want a platform to manage credentials and receive expiration reminders, Intiva Health offers that as well. Visit our credentialing software page for more information.

You can download our provider credentialing checklist here.
There are two ways to fill it out:
- By hand
If you want to fill it out digitally, all you need to do is check each mark by clicking them. After you’re done filling it out, you can either print it out or save it as a PDF. We even have a button you can click to share it with someone else.
If you’d rather do it by hand, simply print out the checklist and fill it out.
Whether you fill it out digitally or by hand, we hope it helps make credentialing faster and easier for your facility.
